This form is required for persons who will help in any way with coaching, are not licensed to teach in Wisconsin, and have not completed an approved Coaches Education Course. This form is not required for student teachers who do not have supervisory responsibilities. It is important for the school to have a written contract for services of a coach not licensed to teach.

USE OF A COACH NOT LICENSED TO TEACH IS CONTINGENT UPON THE FOLLOWING CONDITIONS:

The undersigned agrees to the following conditions in making this request:
1. Our school has a coaching emergency as outlined in Article VII – Coaches Qualifications (page 26 of WIAA HANDBOOK).
2. We believe that the candidate(s) has (have) the necessary qualifications to fill the position(s).
3. It is our intent to fill the position(s) with a teacher or CNLT who has completed a WIAA approved coaches education course within one year.
4. Our request covers the current school year only and if an emergency exists in the future, we understand we must make this application again, but not for the same candidate(s).
5. In the event of a claim against our school and/or the WIAA, because of any actions of this person (these persons), our school district will hold the WIAA harmless and indemnify it (the WIAA) against.
