All portions of this form must be completed in ordered to be considered for assignment to a WIAA Skinfold Measurement Training Workshop. The deadline for turning in this form is June 15, of the year of the training session. Those selected to participate in a Skinfold Measurement Training Workshop will be notified by early August. Unfortunately, not all applicants will be invited to participate in a Skinfold Measurement Training Workshop, as space is limited to 12 persons per workshop.

Selection to participate in Skinfold Measurement Training Workshop is based on several criteria:
- geographical need for Trained Skinfold Measurers in a specific area of the state.
- qualifications of the applicants.
- high school affiliation of applicant.
